What to Bring to Your Appointment

• Your CareCard or BC Services Card (for BC Residents)
  Please note that out-of-province and out-of-country patients will need to provide their medical insurance information.


• A list of all your current medications
  Tell your doctor about any medications you are currently taking. Please include all drugs, and also vitamins and supplements, as they can interfere with treatment.


• Any doctor, X-ray or laboratory reports you have
  If you don’t have these, don’t worry! Please bring the name and telephone number of all the doctors involved in your care, as well as the locations where you have had X-rays or blood tests done.


• A list of questions you want to ask the doctor
  Write your questions on this sheet and bring it to the Doctor. Click Here
